Patient Navigation

In addition to our clinical services, Capital Breast Care Center provides culturally appropriate navigation services for patients with abnormal test results. Capital Breast Care Center Patient Navigators ensure that women receive timely follow-up diagnosis and treatment, providing emotional and logistical support to patients. Patient navigators are dedicated to breaking down barriers to care and ensuring that patients receive the support and knowledge needed to successfully navigate the healthcare system. Navigation is considered complete only when no additional breast health services are needed, or when a patient has completed treatment for breast cancer. 

 

Our Patient Navigators are trained to:

  • Guide patients through the healthcare system
  • Help breast patients without insurance apply for financial aid
  • Arrange mammography services
  • Provide emotional support to patients and their families
  • Explain diagnostic procedures
  • Ensure that follow-up care is available
  • Help patients get appropriate social services
  • Help locate resources and support to meet patient needs


Our Patient Navigation Program:

  • Increases access to breast health services and information
  • Enhances patients’ healthcare experience
  • Reduces the number of delayed or missed appointments
  • Makes health care more efficient
